Summer Camp 2025

The Urban Farm (TUF) is a licensed Childcare Center located in Denver, Colorado.
Per requirements of the license(s) as issued by the City and County of Denver and the State of Colorado, TUF is mandated to collect specific medical and immunization records for all youth participants of its Summer Camp. Links to download each form may be found below.
Required forms for all youth campers:
PHYSICAL / MEDICAL RECORD
A completed physical or medical record must be provided for each camper
Physicals must be dated within one year of camp enrollment and signed by both the parent / guardian and the child’s medical provider

2. MEDICATION APPLICATION AUTHORIZATION
It is highly recommended that all required medications are administered either prior to or after TUF Summer Camp
If ANY medication is deemed necessary as identified in the provided physical and/or General Health Appraisal Form (this includes epipens, ibuprofen, etc.), a Medication Application Authorization form must be completed and returned in order for TUF staff to administer medications during camp
If medications are necessary but you do not wish for TUF staff to administer them while your child is at camp, a Refusal to Provide Medication form must be completed and returned
All campers must provide records of immunizations, as is required by the City and County of Denver and State of Colorado’s childcare license
Records of immunizations must be dated within one year of camp enrollment and signed by both the parent / guardian and the child’s medical provider
If there is no previously completed immunization record available, an Immunization Record Form may be completed and returned; Immunization Record Form located below
TUF will accept a notice of exemption from immunization, so long as the form is one of the provided standard forms available on the Colorado Department of Public Health & Education website
The State of Colorado requires all children at Summer Camp to be adequately protected from the sun
All parents or guardian must complete the form in order to grant TUF permission to apply sunscreen to your child or to deny permission to do so

TUF will accept all forms and forms of the same purpose that are dated and signed by BOTH parent/guardian and the medical care provider within one year of the youth’s last day of camp. Forms may also be completed and turned in physically to TUF Staff upon the first day of camp for each youth.
